What is SEMA-1 (semaglutide)?

Semaglutide is a GLP-1 receptor agonist from Novo Nordisk. It’s 94% identical to human GLP-1, with two changes that matter: an amino acid swap at position 8 that blocks the enzyme DPP-4, and a C18 fatty diacid chain that binds albumin. Together they take the half-life from about two minutes to about a week.

It’s approved as Ozempic and Rybelsus for type 2 diabetes and as Wegovy for weight management, which makes it the most studied compound in the GLP-1 class.

SEMA-1 mechanism of action

GLP-1 is a gut hormone released after a meal. It tells the pancreas to release insulin, but only when blood sugar is high, and it slows how fast the stomach empties. Receptors in the hypothalamus pick it up too, which is where its effect on appetite comes from.

Natural GLP-1 is broken down by the enzyme DPP-4 within a couple of minutes. Semaglutide swaps one amino acid to resist DPP-4 and carries a fatty acid that binds albumin in the blood. Together those changes stretch its half-life to about a week, which is why it’s studied as a once-weekly compound.

What the research says about SEMA-1

The STEP 1 trial (Wilding and colleagues, New England Journal of Medicine, 2021) reported average weight loss of 14.9% at 68 weeks against 2.4% on placebo.

The SELECT trial (Lincoff and colleagues, 2023) followed more than 17,000 adults with heart disease and overweight but no diabetes, and found a 20% lower rate of major cardiovascular events on semaglutide.
